These forms are for the Oregon-Washington Carpenters Trust Funds which covers the Oregon-Southwest Washington area. They may or may not be the same as forms used in other trusts. Carpenters out of work questionaire EIS out of work questionaire Millwrights out of work questionaire
	You need a completed questionaire on file before you can get on the out of work list. We are supposed to fill out and submit a new form each year.



Millwrights Local 204 out of work questionaire	You need Local 204's questionaire on file (with them) if you want to get on the millwright out of work list for the Seattle - Western Washington area.


Change of address form	Use this form to change your address for all Trust funds.

Short term disability claim form	Use this form when you are injured and can't work. Current benefit is $400 per week if hurt off job site and $50 per week if hurt on the job site.


Domestic Partnership Affidavit form	To register your unmarried companion for trust benefits.

Health & Welfare Enrollment & Beneficiary Designation form	To enroll in our Health & Welfare trust and to add-remove dependents.

Health & Welfare Fund Reciprocal Agreement form Pension Fund Reciprocal Agreement form	When you work out of area (Seattle or Spokane for example) the health & welfare and pension monies deducted from your paycheck stay in those area trust funds unless you use these forms. This could result in loss of coverage for health & welfare and loss of pension benefits when you retire. Be sure to print out 3 of each form when you need them. One goes to our Trust Office in Portland, one goes to the trust office for the area you are working in, and one should be kept by you. Keep all of your copies for each time you work out of area...very important.


Vacation Fund Beneficiary form	Just what it says.

Vacation Fund Distribution form	The form to select how and when your vacation check is distributed.

Vacation Fund Request for Additional Withdrawal form	For when you want to withdraw funds but it's not the scheduled time you selected with the form above.

Pre-retirement Pension Beneficiary form	The beneficiary form for unmarried carpenters. It determines who gets your pension if you should die before retirement age.


Pension Trust Estimate of Benefits form	This form will get you started if you are considering retiring.
